This presentation will focus on polypharmacy in the elderly population and examine how the use of multiple medications can significantly impact patient safety and overall quality of care in older adults. It will explore common contributors to polypharmacy, including the presence of multiple chronic conditions and fragmented care, as well as associated risks such as drug–drug interactions, increased hospitalizations, and medication burden.

 

The presentation will also highlight the critical role pharmacists play in identifying duplicate therapies, conducting comprehensive medication reviews, and collaborating with healthcare providers to optimize therapy through deprescribing and individualized treatment plans.  Polypharmacy is a topic that highlights the importance of pharmacist-led interventions in enhancing the safety of medication use in the geriatric population.
